Request Baptism Certificate

  • MM slash DD slash YYYY

  • Full Name of Baptized Person as it appears on the birth certificate

  • Requirements:

    • the original Birth certificate for the child or adult. A copy will be made here and the original returned to the parent or adult requestor.
    • a photo ID of the parent or adult requestor. A copy will be made here at the time of request.
    • the same individual and photo ID presented at the time of pick-up.

    Certificates for adults will only be released to the adult on the certificate or the church requesting the certificate.

    Please be advised, fraudulent documents will be reported to the appropriate authorities.

    (An individual must be baptized at St. Andrew the Apostle or St. John the Evangelist parishes for a certificate to be prepared. We do not prepare baptism certificates for other parishes)
